Key Responsibilities:

• Provide skilled nursing care in accordance with care plans and physician orders

• Assist with medication administration and wound care

• Monitor patient conditions and report any changes to the RN/Clinical Director

• Collaborate with interdisciplinary team members to ensure quality outcomes

• Educate patients and caregivers on treatments and wellness practices

• Document care and observations accurately in the EMR system

• Communicate with physicians, case managers, and other team members as needed

Qualifications:

• Current LPN license in the state of Indiana (required)

• Minimum 1 year of home health, hospice, or clinical nursing experience preferred

• Strong assessment, documentation, and time-management skills

• Reliable transportation and valid driver’s license (for field visits)

• Passion for helping others and making a difference in patients’ lives

• EMR experience a plus (Axxess preferred but not required)
