Occupational Therapist | Home Visits | Per Diem
Join our outstanding team of home health professionals as an Occupational Therapist, serving the vibrant communities of Allegan and Ottawa county.
Atrio Home Care, a partnership between Holland Home and Resthaven, is looking for a compassionate Occupational Therapist to join our growing Holland/Lakeshore team!
This position develops and implements occupational therapy treatment plans for assigned home health clients. Home health clients are clustered geographically within our coverage area to avoid unnecessary travel as much as possible. This position includes providing direct care in North and South Holland, with coverage extending west to Hudsonville and as needed to Grand Haven and south toward Allegan.
Responsibilities:
Conduct initial and ongoing occupational therapy assessments.
Participate in discharge planning and complete OASIS assessments.
Develop treatment plans in consultation with physicians and care team members.
Assess and coordinate the acquisition of necessary adaptive and other medical equipment.
Provide occupational therapy to restore or maintain ADL function, home safety assessments, and upper extremity coordination/strength.
Identify barriers to desired outcomes and evaluates outcomes of care.
Supervise and guide assigned home health aide.
Prepare clinical documentation, progress notes, and other required documentation.
Instruct clients, families, and caregivers on therapeutic techniques.
Address and resolve client and family concerns related to therapy.
Essential Skills and Abilities:
Excellent verbal and written communication skills.
Effective time and schedule management skills to consistently meet critical deadlines for charting.
Proficiency with technology: tablets, mobile phones and EMR software.
Location: This position will involve providing direct care to clients in the North and South Holland areas, extending West to Hudsonville.
Schedule/Shift
Day shift; flexible schedule
No weekends or On-call

Qualifications
Occupational Therapist in the State of Michigan
CPR certification
Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits
1 year post-graduate OT experience in a health care setting
Passion for delivering high-quality care to older adults
