    Job Details

  • Provide nursing care for patients following established standards and practices in accordance with the South Carolina Nurse Practice Act.
  • Apply professional nursing theory and practice to assess, plan, implement and evaluate patient care.
  • Plan, organize and direct hospice care.
  • Supervise and coordinate care to patients with terminal illnesses, diseases and compromised health states.
  • Provide comprehensive skilled nursing services to qualified patients in their homes or hospice inpatient setting.
  • Work with minimum daily supervision while performing interdependent practice in the field.
  • Supervise hospice aide, administer medications, symptom assessment and management, education/teaching, and end-of-life care.
  • Communicate with physicians regarding patient needs and report changes in patient condition.
  • Provide and maintain a safe environment for the patient.
  • Teach self-care techniques and ensure arrangements for equipment or necessary services.
  • Assess patient condition, administer medications, and evaluate/treat symptoms and end-of-life needs.
  • Complete, maintain and submit accurate clinical notes/documentation.
  • Attend interdisciplinary group meetings and participate in quality assessment performance improvement teams.
  • Assume responsibility for personal growth and development through continuing education and in-service classes.
  • Supervise care provided by LPN and Hospice Aides and delegate responsibilities when required.
  • Coordinate with other disciplines, agencies, families to ensure patient care needs are met.
  • Participate in on-call duties as defined by policy.

    Job Requirements

  • Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.
  • Current CPR certification.
  • Must have a valid South Carolina driver’s license with a good driving record to make home visits.

    Schedule Information

  • Occasional on-call evenings and weekends based on the needs of the department.

  • Ability to implement the nursing process and principles of teaching and learning.
  • Good organizational and time management techniques with ability to independently prioritize workload.
  • Excellent observation, problem solving, and verbal and written communication skills.
  • Competency and verification of skills may be required when applying for specialty practice.
  • Frequent standing, walking, bending, crouching, stooping and driving.
  • Frequent lifting/moving patients, intermittent lifting/moving or carrying up to 50 lbs.
  • Frequent pushing or pulling over 50 lbs.
  • Frequent use of finger/hand dexterity and eye/hand coordination.
  • Constant reaching with hands/arms.
  • Frequent talking or hearing; corrected hearing and vision to normal range.
  • Exposure to blood, body fluids or tissue and communicable diseases.
  • Exposure to toxic substances, pharmaceutical preparations, hazardous and unsafe driving conditions.
  • Intermittent exposure to risk of blood-borne pathogens.
  • Moderate noise level in work area.
  • Work under stressful conditions, deadlines, or irregular hours.
