\n\nLMSW Job Requirements\n

\n
• Active Licensed Master Social Worker (LMSW) license in Virginia or compact state (required at submission)\n
• Minimum 2 years of clinical experience in an acute hospital setting\n
• BLS and ACLS certifications (required)\n
• Proficiency with Meditech EMR and CarePort Care Management documentation systems\n
• Knowledge of AIDET patient experience standards\n
• ACMA (American Case Management Association) or CCM (Certified Case Manager) certifications preferred\n

\n\nSocial Worker Responsibilities\n

\n
• Conduct initial patient evaluations and reassessments for discharge planning\n
• Coordinate referrals for post-acute healthcare resources and placements, including home health, DME, home infusion, hospice, skilled nursing facility (SNF), and long-term care\n
• Arrange discharge transportation and facilitate smooth transitions of care\n
• Document all hospital case management activities using Meditech and CarePort Care Management systems\n
• Float within the Case Management Department, supporting ER, NICU, ICU, PCU, and Med-Surg units\n
• Collaborate with Case Management Assistants, Managers, and Directors to address patient needs\n
